Patients will be allocated a "named GP", although you will be able to see any of the doctors within the practice by appointment.
The practice area covers Pendlebury, Clifton and Swinton North only. The practice does have existing patients in surrounding areas but we will only be taking on new patients in the practice area.
If you are homeless you are welcome at our practice, you will have no issues in registering at the practice. You may be asked for identification, if you do not possess any, don't worry we will register you anyway. If you do not have an address you can use we will register you at the practice address.
We will accept reverse charges if you need to telephone us and do not have access to a telephone or funds to do this.
If you are referred for further treatment, we will ensure we do the referral booking whilst you are at the practice, you should not leave the practice without your appointment.
If you have investigations at the practice we will give you a follow up appointment so that you can receive the results of your investigations.
We have a welfare rights adviser working at the practice one day a week, we will ensure that you get an appointment if you need one.
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. You will need to come into the practice to sign your Pre-registration Form form before the practice will register you.When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
