Health Promoters

Every month the practice will be leading on various health promotion campaigns where we will have patient information available for you, health promotion displays in the surgery and we will be encouraging patients to engage with us in helping you to help yourself.

Telephoning the Practice

We are asking all patients who are ringing for results of investigations, queries with prescriptions, general queries to telephone the practice between 11am - 4.00pm - this means the telephones will be available for those patients who need to see a doctor or nurse that day and also for patients ringing in requesting a visit.  Thank you in advance for your cooperation.


Advanced Nurse Practitioners

This practice employs ANP's (Advanced Nurse Practitioners) who are highly qualified individuals who work in a similar way to the GP's, seeing, assessing and sometimes referring patients.   


Booking Appointments

Please be aware when booking an appointment the GP's are only given a 10 minute slot for each patient unless it is a non-English speaking patient who has an interpreter or a patient who may require more time because of a disability. 

If you feel you may need more than a 10 minute slot please inform the receptionist of this.  If you do attend an appointment with a number of health problems the GP may ask you to rebook another appointment so that more time can be spent with you. 

If you wish to register for online appointment booking, please speak to the receptionist or contact the practice to be sent your registration details.


Extended Surgery Hours

The practice now offers an extra 6 hours surgery time per week, some weeks this may be a Saturday morning from 9.00am – 12.00pm or a Weekday morning from  7.00am – 8.00am and Wednesday Evening 6.30pm – 7.30pm.

If you work and find it easier to book an appointment out of work times, please feel free to book an appointment in our extended surgery hours.

Welcome to Silverdale Medical Practice.  As a patient of Silverdale Medical Practice you are able to see any of the GP’s in the practice, you no longer have an individual doctor and are registered with the practice. 

As a patient of Silverdale Medical Practice you can expect from our team:- 

  • That we are committed to providing you with the best possible service. 
  • You will be treated as an individual and will be given courtesy and respect at all times, irrespective of your ethnic origin, religious beliefs, personal attributes or the nature of your health problem. 
  • You have the right to see your health records, subject to any limitations in the law.  We are committed to ensuring that your medical records remain confidential. 
  • We shall give you full information about the services we offer.  Every effort will be made to ensure that you receive the information, which directly affects your health and the care being offered. 
  • We shall endeavour to meet your requests for appointments at times to suit you where this is possible. 
  • If you need an urgent appointment, please make this clear to the receptionist and we will try to fit you in with a doctor the same day, if your condition is not deemed urgent by the GP, you may be asked to rebook in a routine appointment. 
  • We shall process your repeat prescriptions within 48 hours of your request, (delays may be caused if a patient has not attended for reviews when required). 

We are constantly trying to improve our service.  If you have any suggestions or comments about any aspect of our service please inform our Practice team.

Patient Responsibilities   

  • We ask you to be courteous to the staff at all times (staff are working under strict instruction from the doctors).  We operate a zero tolerance policy towards verbal and physical abuse, if you are abusive to the staff or doctors you may be asked to register with another practice.  Shouting and talking over people is not acceptable. 
  • We ask you to attend appointments on time or give the practice adequate notice if you wish to cancel - lateness or non-attendance causes inconvenience and increases appointment-waiting times for other patients.  If you fail to keep an appointment without notifying us you will be sent a letter, if you fail to attend on 3 occasions you may be asked to register with another practice. 
  • We ask you to remember that an appointment is for one person only and that if another member of the family needs to be seen or discussed, another appointment should be made.                                                     
  • We ask that you do not book an urgent appointment (same day) for something which is not urgent, if you do the doctor may ask you to rebook a routine appointment.  The production of a fit note is not urgent. 
  • We ask you to make every effort to attend the surgery when possible to make the best use of medical time - home visits should be medically justified and not requested for social convenience.  Out of hours calls (after 6.30pm and before 8.00am Monday - Friday and weekends), should only be requested if they are felt truly necessary.  The Out of hour’s telephone number is NHS 111. 
  • The service out of hours and the accident and emergency department is for urgent cases. If your case is not urgent you are better served waiting to see your own doctor the next day.  People with serious illnesses need to be seen promptly out of surgery hours, adding to their waiting time with non-urgent requests prevents them getting early care.                                                                                  
  • Prescriptions – we do not accept prescription requests over the telephone except for the elderly or housebound. It is your responsibility to ensure you have sufficient medication; all but the most urgent requests will be produced within 48 hours.   Requests can be made on the repeat slip on your prescription and placed in the post boxes provided, requests can be emailed using our website www.pendleburyhealthcentre.co.uk or fax:- 0161 212 6566 or alternatively you may email you prescription requests to www.silverdalemedicalpractice@nhs.net . 
  • We ask that you keep the practice up-to-date with your contact details and inform us if you are a carer or are cared for by someone, it is also important that we have your next of kin details especially if you live alone. 
  • We ask that you attend for your reviews when asked, if you receive a letter to attend for your review we expect you to play an integral part in your own health.  The amount of medication you are prescribed may be temporarily reduced if you are not attending for your review.
